Northstar Healthcare is a healthcare treatment platform operated by Telescope LLC. We coordinate medical care for our patients and manage appointment scheduling, clinical intake paperwork, and treatment-related communications. References to "Northstar Healthcare" in our text messages and on this site refer to services provided by Telescope LLC.

Our Messaging Program

Through the Northstar Healthcare patient messaging program, we send transactional, healthcare-related text messages to patients who opt in. This is not a marketing program. Message types include:

• Appointment reminders and confirmations
• Missed-visit and treatment-compliance notices
• Check-in notifications
• Secure links for patients to complete or resume required clinical intake paperwork
• Telehealth visit links

Message frequency varies. Message and data rates may apply.

For help, reply HELP to any message or contact us at support@telescope-cm.com. To stop receiving messages, reply STOP to any message at any time; you will receive a one-time opt-out confirmation and no further messages.

How You Opt In

Consent to receive text messages is optional and is not a condition of receiving care, using our Services, or completing any form. You can opt in through either of the following methods:

• Patient intake web form. On our public patient intake form at telescope-cm.com/patient-intake, after entering your phone number you may check an optional consent box (unchecked by default). The form can be submitted whether or not the box is checked.
• In person or by phone. When staff complete your intake in person or by phone, the same disclosure is provided in writing and verbally, and your consent is recorded.

The consent box on the intake form reads:

"I agree to receive appointment reminders, check-in notifications, and other healthcare-related text messages from Northstar Healthcare at the phone number provided. Consent is optional and not a condition of any purchase or service. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time. See our Privacy Policy and Terms of Service."

Privacy

We do not sell, rent, or share your mobile phone number, SMS opt-in information, or messaging consent with any third parties or affiliates for their own marketing purposes. See our full Privacy Policy and Terms of Service for details.
